Abstract
Title: A decade analysis at an academic hospital center of the Transjugular Intrahepatic Portosystemic Shunt (TIPS) procedure with Intracardiac-echocardiography (ICE) guidance.

Background: The TIPS procedure has been well established since 1989 and has undergone developments with the utilization of Intracardiac-echocardiography (ICE) guidance. The aim of the study is to analyze the utilization of ICE for TIPS procedures.
Methods: A retrospective chart review was conducted, analyzing all TIPS procedures completed at Henry Ford Hospital from January 2010 to January 2024. TIPS procedures were divided into procedures utilizing ICE (ICE group N=231) and without ICE (conventional group N=116).
Results: No statistically significant difference in radiation dose between the ICE group (mean = 962.4 ± 875.0 mGy) and the conventional group (mean = 1114.8 ± 856.0 mGy) (P = 0.06). No statistical difference in the average fluoroscopy time between the ICE group (mean = 42.0 ± 26.6 min) and the conventional group (mean = 40.6 ± 21.8 min) (P=0.825). The delta gradient was found to be not significant between the ICE group (mean = 9.8 ± 5.6 mmHg) and the conventional group (mean = 9.0 ± 5.1 mmHg) (P=1.89). Non-traditional connections were performed more frequently in the ICE group (45.2%) than the conventional group (25.8%) (P = < 0.001).
Conclusion: Our study demonstrates that ICE leads to increased creations of shunt connections that do not follow the traditional method.
