Abstract
Background
Torn Anterior Cruciate Ligaments (ACLs) are common injuries for professional football players with significant impacts on future performance and career longevity. This study aimed to explore the quality of return to performance after ACL reconstruction in National Football League (NFL) players with fantasy football scores as an evaluative metric of recovery length, performance, and long-term outcomes.
Methods
We analyzed 200 NFL players who received ACL reconstruction surgery between 2005 and 2021. Players were evaluated based on pre- and post-injury fantasy scores, recovery time, and career length. Data were collected from public sources and performance was evaluated seasonally.
Results
71% of players who underwent reconstruction surgery played in a future regular season NFL game. In their post-injury career, only 19% of players achieved or exceeded their pre-injury fantasy performance levels. This figure grew to 27% when considering only the 71% of players who returned to play. 79% of players who reached or exceeded their pre-injury fantasy performance levels did so within 2 seasons following their injury. Players with higher pre-injury fantasy scores demonstrated faster recovery, better post-injury performance, and longer careers compared to those with lower scores. Younger age and fewer professional seasons played prior to injury were also associated with more successful recoveries.
Conclusion
Fantasy scores provide an informative and quantifiable metric for assessing the quality of return to play (RTP) in NFL players following ACL reconstruction. Higher pre-injury fantasy scores are associated with better recovery outcomes, indicating their potential utility in guiding RTP decisions and rehabilitation strategies.
